About this Transfer Syntax

DICOM Transfer Syntaxes define three things — byte ordering (little vs big endian), VR encoding (implicit vs explicit), and any pixel data compression (JPEG, JPEG 2000, RLE, MPEG, etc.). Two systems negotiate a Transfer Syntax when they establish a DIMSE association, and DICOMweb STOW-RS advertises supported ones via Accept headers.

This Transfer Syntax is retired from the current standard. Legacy systems may still request it; new associations should negotiate an active alternative.

Working with Native — Explicit VR Big Endian

Transfer Syntax conversions sit at the heart of most integration engines. Saga IT's medical imaging team builds production DICOM gateways that negotiate, transcode, and route across Transfer Syntax boundaries — including cloud DICOM services that only accept specific families. Heavy compression workflows (JPEG 2000 ↔ native, for instance) often benefit from a Mirth Connect staging layer.
